Critical Note: Procore does not support the newly introduced High Compression Image and Video formats. Photos should be saved in JPEG / JPG format. Photos saved in the High Compression / High Efficiency formats become stuck in the Procore App Upload Queue and will fail to upload. Instructions for configuring an iPhone / iPad to save in the correct format can be accessed here.


  1. Launch the PROCORE app on your mobile device and sign in as needed. The app will open the most recent project you were viewing, or prompt you to select a project if you have not previously used the app.


  2. Tap on the name of the project you are currently viewing to select a different project from the list of available projects or to search by project name or number.
     
    • For U.S. Operations (Landmark Structures I LP)
      • Select the specific project for which the SER should be completed.
      • Select the EHSNPJ (EHS Non Procore Project) project for legacy jobs which are not managed through Procore.
      • Select the EHSNPJ (EHS Non Procore Project) project for SERs performed at the Fort Worth or Wheaton office locations.
      • Select the FAB project for SERs performed at the Decatur, TX office location.

    • For Canadian Operations (Landmark Structures Co, Landmark Municipal Services)
      • Select the specific project for which the SER should be completed.
      • Select the EHSNPJ (EHS Non Procore Project) project for legacy jobs which are not managed through Procore.
      • Select the EHSNPJ (EHS Non Procore Project) project for SERs performed at the Burlington office location.
      • Select the SHOP project for SERs performed in the Burlington Shop facility.


  3. Tap on Incidents tile to launch the project specific inspections tool. A list of Incidents which are present on the job, and viewable to you will be displayed.




  4. Tap on the plus ("+") button to create an Incident Report. The Create an Incident form will display for you to fill out basic details.



     

  5. The following fields are required...

    • Title should be a short description of the Incident. The title SHOULD NOT include detailed information regarding the incident. Some examples are Flat Tire, Tripping Hazard due to Mud Ruts, Stolen Tools, Injured Finger.

    • Attachments should include any documentation related to the incident. This may include documents, images, drawings or other items.
      • Note: Images which show medical details SHOULD NOT be attached to the Incident. These may be added later to an Injury Record or collected by EHS staff during incident investigation.

    • Description should contain a narrative description the incident including what happened including what led up to the incident. This should not include medical details of any injuries. Specific details will be captured in another location.

    • Accident Type should be chosen from the list of available options. 

    • Phase should be chosen from the list of available options.

    • Name of Person Supervising Activity should be a the name of a person, manually entered.

    • Person(s) Involved should be a short list of the names of people involved in the accident, manually entered.

    • First Aid Given To should be the name of any person who received First Aid.

    • Location field should be set to the appropriate location.
      • For EHSNPJFAB, and SHOP projects select the appropriate Job from the location list.

      • For all other projects, leave the location field blank.


  6. The following fields are optional.

    • Accident Location should be the specific location where the incident occurred on or off the jobsite.

    • First Aid Given To should be the name of any person who received First Aid.

    • Police Report should be turned on (green) or off (grey) to indicate a Police Report was filed.

    • Police Report Number should contain the police report number.

    • Police Report Attached should be turned on (green) or off (grey) to indicate the Police Report is included in the attachments.

    • Name of Police Officer should contain the name of the Police Officer who responded to the scene.

    • Phone Number of Police Officer

    • Citations Issued should be turned on (green) or off (grey) to indicate a citation being issued.

    • Citation Issued to Whom


  7. Tab on the Save button to save and submit the Incident. The Incident Details screen will load where you can make adjustments to the entered data.




  8. You may now add Witness Statements to the Incident by scrolling down to the Witness Statements section and tapping on the Add button.




  9. Enter the witness details including Witness Name, Date Received, Statement, and Witness Phone Number. Tap the Save button to save the witness details when complete.

    • You may choose a Witness from the Procore Directory or add a new person by clicking on the Add New Item button.

       


    • You may attach witness statements in image, PDF, or standard document formats.

    • You may optionally record a witness statement directly within the app.


  10. The following fields should not be changed when submitting an Incident. These fields are for EHS Staff use only and may be modified during the Incident Investigation process.

    • Recordable. This field should always be left in the Unchecked / Grey / Off state unless set by EHS staff.

    • Private - This field should be left in the Checked / Green / On state unless set by EHS staff.
